Why we ask for this, and how it is used
  • Legal name and UEI — matched against your SAM.gov registration. An application cannot be submitted while the registration is inactive.
  • Organization type — determines eligibility and the required local match. Cities, counties, and tribal governments apply directly; private owners apply through their state.
  • Business address — used for official correspondence about this application only. It is not shared outside the department.
  • Your entries are saved as you move between steps and kept for 60 days after your last visit.
  • Required fields are marked with an asterisk. Everything else is optional and can be left blank.
