Delivery
How should we send the permit?

Gate codes, a loading dock, or the name of the person who signs for mail.

Why we ask for this, and how it is used
  • Mailing address — where the printed permit is sent. It does not have to match your billing address, and it is not shared with any other agency.
  • Delivery method — first-class mail is included in the permit fee; express courier is charged at the payment step and needs a street address, not a PO box.
  • Email — used only to send the confirmation number and delivery updates for this application. You will not be added to a mailing list.
  • Saving and finishing later keeps this application for 30 days; after that the entries are deleted.
  • Required fields are marked with an asterisk. Everything else is optional and can be left blank.

Save and finish later?

We will keep this application for 30 days. We will email you a link to come back to it.
